I began this novel on my first visit to Chicago, and finished it as a resident here. I'd like to thank everyone who helped in the transition: on the UK end, Bob, Gwyn, Pip, Steve, Alma, my GP and my Mum; on the US end, my wife, my in-Âlaws, my new friends and colleagues.
