   Cushing syndrome especially caused by ectopic ACTH production

   Hyperthyroidism (especially in Asian persons)
   Drugs
   Diuretics (e.g., thiazides, ethacrynic acid, furosemide); assay for diuretics should be done if urine chloride >40 mmol/L
