In the United States, the great mountaineer Tom Hornbein advised me on high-altitude physiology. I am grateful also to Duane Raleigh and the editors of

who published my initial article about the Annapurna controversy, and to Tom McCarthy, an editor at International Marine and Ragged Mountain Press (a division of McGraw-Hill), who first urged me to write a book about it.
